Team Manager and Team Volunteer Requirements
All team manager and team volunteers (ie Locker Room Monitors, Off-ice Trainers, etc) are required to complete the following:

Team Fundraising Notification Request
DYHA Board of Directors and Administration request your notification of all team fundraisers to confirm that there are no scheduling conflicts with DYHA association fundraisers and to ensure we are ready to track all funds raised for deposit into the team account.
Please click on the logo to email fundraising@dyha.org with your request. Requests will be reviewed promptly and you will receive a confirmation from staff. Please include:
1. Your name and phone number
2. Date of your fundraiser
3. Expected date for team account deposits of funds raised
4. Brief description of what you plan to sell or the event
